Assessment and follow up:    $130.00/ Hour 

Official Written Assessment:    $50.00

We are unfortunately unable to offer direct billing to insurance providers though most will cover OT services through client submission. 

Payment can be made by cash or e-transfer to sheilafarnell@purpose-fullplayot.org

Please note that payment is due on the date of service. 

How old does my child have to be?

Currently we offer services to children ages 1 year to 15 years. If your child falls outside of these parameters please contact us via email to discuss options.
